CASE MANAGEMENT

Case Management  is a tool used to promote strategies and initiatives to improve the health status of employees, reduce health costs for employers and reduce overall staffing costs incurred with employee absences.  Case Management  brings uncoordinated efforts together.  It cuts across corporate and medical departments by allowing simultaneous communication.  Fairweather’s Case Management program is supported and staffed by CarePlan ♥ Case Management Service’s Registered Nurses.  CarePlan ♥ is an Alaska based company with a thirteen year history of serving many of Alaska’s largest companies. 

Our Goals:

· Early notification, intervention, and case management to reduce the duration of employee illnesses and injuries

· Validate and monitor medically necessary work restrictions and accommodations

· Consistent and efficient communication between all disciplines

· Reduction in costs through coordinated services

· Reduced “time away from work” cases through proper management

· Referral for a physician capacity evaluation for ADA and “fit for duty” cases

· Decreased fragmentation of care and services for the employee

· Enhanced collaboration among disciplines in an efficient manner

3. Utilizing Physician Staffed Clinics in Anchorage and Kenai with a comprehensive understanding of Aggressive First Aid Guidelines

4. Coordination of Lodging/Appointments for employees who reside outside of Anchorage
